Rapid malaria antigen tests, or Rapid Diagnostic Tests (RDTs), are quick, lateral flow immunoassays that detect specific proteins (antigens) produced by malaria parasites in a drop of blood. They provide results within 15–20 minutes, making them vital for diagnosing active infections, particularly Plasmodium falciparum, at the point of care.

Key Aspects of Rapid Malaria Antigen Tests

  • Interpretation: A single line at 'C' (Control) is negative, while two or three lines (including at 'T') indicates a positive result.
  • Performance: RDTs are generally accurate and crucial where microscopy is unavailable, though they can sometimes produce false results if read too early, too late, or if antigen levels are low.
  • Benefits: They enable rapid treatment, particularly in endemic areas with limited laboratory infrastructure.

Procedure: A small blood sample is taken via finger-prick, mixed with a buffer solution, and added to the test cassette.

  • Mechanism: They use monoclonal antibodies to detect parasite proteins like HRP-2 (

𝑃.𝑓𝑎𝑙𝑐𝑖𝑝𝑎𝑟𝑢𝑚) or pLDH (𝑃.𝑣𝑖𝑣𝑎𝑥 and others), providing a colored line in the result window.

Limitations

  • They are qualitative, not quantitative, meaning they don't tell how high the parasite count is.
  • Certain tests may not detect all malaria species equally well, or might remain positive for a period even after successful treatment (especially HRP-2 tests).
